- A list of your current medicines, including doses
- Any referrals, test results or hospital letters
- Your child's blue book for children's appointments
- Concession or health care card, if applicable
Cancelling or changing an appointment
As a courtesy to other patients, please let us know as early as you can if you cannot attend. This allows us to offer the time to someone else who needs care. You can cancel by phoning the clinic or through your HotDoc booking.
Telehealth at Emu Plains
Telehealth appointments are available for existing patients of our Emu Plains clinic on Mondays and Wednesdays from 9:00 am to 1:00 pm. Phone (02) 4735 2153 to arrange a telehealth appointment.
Prescriptions and referrals
An appointment is usually required for repeat prescriptions and referrals so your doctor can review your care safely.
